While we will have forms available to fill out at auditions, it would be helpful to have the forms completed prior to arriving for your audition time slot. If it is not possible to complete the forms ahead of time, please try to arrive early enough to allow for paperwork.
You will need one audition form per student auditioning. One Family Information Form covers multiple students within the same family.
